Jan. 12, 2018 – The Pharmacy Department at AxessPointe Community Health Centers, Inc. scored a four out of four on OutcomesMTM 2017 performance ranking, and is performing in the top 2% of all U.S. pharmacies participating in the program.

Medication therapy management (MTM) is a process where pharmacists analyze a patient’s medication regimen by doing a complete medication review (CMR) and assessing four main factors: indication, effectiveness, safety and convenience. OutcomesMTM scores participating pharmacies on CMRs and Targeted Intervention Programs (TIPS), which are the individualized recommendations by the pharmacist that the patient’s physician accepts and implements.

This process has shown that patients who receive medication therapy management are more likely to take their medications and in most cases, have better control of their diseases.
“This great performance reflects the hard work of our pharmacy staff and our medical providers’ belief in collaboration between the two disciplines to achieve the best outcomes for our patients,” stated Dr. Magdi Awad, Director of Pharmacy for AxessPointe.

OutcomesMTM is the nation’s leader in the design, delivery and administration of medication therapy management programs. The company was founded in 1999 by two Iowa-based pharmacists and is connected to over 5.5 million patients through its nationwide network.

AxessPointe Community Health Centers is a federally qualified health center (FQHC) with five current sites in Northeast Ohio, including three in Akron, one in Kent and one in Barberton. An FQHC is a non-profit corporation that delivers primary medical, dental and preventive health services in medically underserved areas. AxessPointe also provides pharmacy, women’s health and behavioral health services.
