Know Your Rights As a Patient

AxessPointe Community Health Centers, Inc. (API) providers and staff are responsible for educating patients on their rights and responsibilities. Providers and staff are also obligated to respect these patient rights and responsibilities.

Patient Bill Of Rights

  • The right to receive services and participate in activities free of discrimination on the basis of race, ethnicity, age, color, religion, gender, national origin, sexual orientation, gender identity, physical or mental handicap, developmental disability, genetic information, human immunodeficiency virus status, or in any manner prohibited by local, state or federal laws.
  • The right to be treated with respect, consideration, dignity, and be provided quality services, appropriate health care, and timely delivery of health care.
  • The right to participate in decisions involving their health care.
  • The right to receive information in language and terms appropriate for the patient’s understanding.
  • The right to obtain from their caregivers relevant, current, and understandable information concerning their diagnosis, treatment, and prognosis.
  • The right to confidentiality of health records and health information as required by law.
  • The right to be informed of how AxessPointe uses their confidential health information in accordance with the law.
  • The right to auditory and visual privacy during all health care interviews, examinations, and treatments.
  • The right to voice complaints or concerns about AxessPointe services.
  • The right to give informed consent to or to refuse any service, treatment or therapy, including medication absent an emergency.The right to be informed if a student doctor/resident is seeing them under the guidance of a doctor; and the right to refuse to be seen or examined by a student.
  • The right to refuse participation in experimental research.
  • The right to formulate advance directives, submit them to AxessPointe staff, and rely on practitioners to follow them when within the parameters of the law.
  • The right to participate in AxessPointe’s sliding fee discount scale program if eligibility requirements are met.
  • The right to be treated in a clean and safe environment.
  • The right to be informed of their rights and responsibilities.

Your Responsibilities As A Patient Are:

  • The right to be informed of their rights and responsibilities.
  • The responsibility to treat staff and other patients with respect.
  • The responsibility to be on-time to attend and give reasonable notice if unable to attend.
  • The responsibility to provide complete and accurate information to the best of their ability, including medications, personal and family history, and any allergies or sensitivities.
  • The responsibility to ask questions and inform staff if they do not understand the information that is being given.’
  • The responsibility to pay their bills when services are rendered.
